• the filled in application form for the Copyright Registration Certificate;
• the copy or sample of work;
• the power of attorney signed for the administration of application, in due form;
• additional documents (if required);
• the service charges including the relevant state fees and duties, as well as accrued fees.

• the copy of the application form filed to the UACRR.
